Add Initial 1-36A to Veterinarian Record or Add Additional 1-36A to Veterinarian Record
The title of this VSPS application page depends on what task you are doing:
If you are submitting a veterinarian's initial accreditation data - this page is titled Add Initial 1-36A to Veterinarian Record.
If you are submitting other types of changes to a veterinarian's record - this page is titled Add Additional 1-36A to Veterinarian Record.
Whichever page you are on, you will see and use the same VS 1-36A Form. The procedure for using this form appears below.
Important: Your VSPS user role determines which Vet Accreditation Module pages, forms, features, and commands you will see and can use.
NOTES:
Fields marked with an asterisk (*) are required.
Some drop-down lists allow you to select multiple items by pressing and holding the CTRL key while highlighting each item.
Dates can usually be entered in two ways: using the Calendar
, or typing them manually in the mm/dd/yyyy format.
Phone numbers: VSPS defaults to the U.S. country code of (1). If you are entering:
a U.S. number, follow the country code of (1) with 10 digits.
a non-U.S. number, be sure to include the correct country code.
If your VSPS role is for a veterinarian, access this page:
On the VSPS Home page, log into VSPS.
From the left menu, select Vet Accreditation.
In the Vet Accreditation Module, click My Veterinarian Record.
The My Veterinarian Record page appears and displays your veterinarian record.
Click the Apply Online button.
A VS 1-36A Form appears.
For instructions on using this form, scroll down to the Completing the VS 1-36A Form section below.
If your VSPS role is not for a veterinarian, access this page:
On the VSPS Home page, log into VSPS.
From the left menu, select Vet Accreditation.
In the Vet Accreditation Module, click Find Vet.
Use the Find Vet page to enter search criteria; then click the Search button to execute a search for a veterinarian.
In the search results list, click [Add 1-36A] in the Action column next to the veterinarian whose record you want to view.
A VS 1-36A Form appears.
For instructions on using this form, scroll down to the Completing the VS 1-36A Form section below.
This form lets you apply the following six changes to a veterinarian record:
Enter a veterinarian's initial accreditation data.
Request authorization for a veterinarian to practice in a new State.
Change a veterinarian's accreditation category.
Enter a veterinarian's accreditation renewal data.
Enter a veterinarian's post-revocation re-accreditation data.
Update a veterinarian's contact information (including name changes).
On a single VS 1-36A Form, you can make up to two changes - updating a veterinarian's contact information, and a second change of your choice.
Important: For each change, you need to fill out only some (but not all) of the fields on the form. The procedure for each change tells you which fields you need to complete.
Scroll down this page to find the procedure for the specific change (each one is underlined) that you want to make.
Enter a veterinarian's initial accreditation data - Fill out only these fields:
Initial Accreditation - Is auto-filled with Yes.
State - Select the State in which the veterinarian wants initial authorization to practice.
License Number - Enter the number issued by this State that appears on the veterinarian's license.
Choose Accreditation Category (Block 15 or 16) - Select Yes in the drop-down list.
Personal Information block - At minimum, complete all the required fields in this block. (Note: For help on these fields, see the Update a veterinarian's contact information, Step 2 section below.)
Accreditation Category Selection block - Select Yes next to either Category I animals or Category II animals.
Contact Information block - At minimum, complete all the required fields in this block. (Note: For help on these fields, see the Update a veterinarian's contact information, Steps 3-4 section below.)
Species Category * - Select 1 or 2 animal categories that the veterinarian's practice focuses on. (Note: To select multiple items, press and hold the CTRL key while selecting each item. To verify your selections, click the Show button.)
Species Codes * - Select 1, 2, 3, or 4 codes for the animal species that the veterinarian specializes in treating. (Note: To select multiple items, press and hold the CTRL key while selecting each item. To verify your selections, click the Show button.)
Primary Medical Discipline - Select the discipline in which the veterinarian specializes.
Employment Type * - Select the workforce category to which the veterinarian belongs.
List of State License block - This block may be empty or auto-filled with data from other VSPS forms.
Authorization Date - Click
to specify the date on which an Area Veterinarian in Charge (AVIC) signed the VS 1-36A Form that approved a veterinarian's accreditation to practice in a State.
Renewal Date - Is auto-filled by VSPS with a date three years from the date that all required fields on the VS 1-36A Form were completed.
Elect to Participate - Select No in the drop-down list.
Verify that you have filled out every required field on the form.
Click the Save New 1-36A Form button.
These messages appear: The veterinarian record was saved. The accreditation request was successfully created.
Request authorization for a veterinarian to practice in a new State - Fill out only these fields:
Authorization in a new state - Select Yes in the drop-down list.
State - Select the additional State in which the veterinarian wants authorization to practice.
License Number - Enter the number issued by this State that appears on the veterinarian's license.
List of State License block - This block lists one or more State licenses granted to the veterinarian. Data in this block is auto-filled by VSPS whenever the veterinarian receives initial or additional accreditations. If you uncheck a box in the License to Practice column, a warning message asks if you want to replace the License Number value with an LAP code (Legally Able to Practice) instead.
Authorization Date - Click
to specify the date on which an Area Veterinarian in Charge (AVIC) signed the VS 1-36A Form that approved the veterinarian to practice in this additional State.
Verify that you have filled out every required field on the form.
Click the Save New 1-36A Form button.
These messages appear: The veterinarian record was saved. The accreditation request was successfully created.
Click the Return to Find Vet Results button. In the results list, this veterinarian now has the newest State code displayed in the Accredited States column.
Change a veterinarian's accreditation category - Fill out only these fields:
Choose Accreditation Category (Block 15 or 16) - Select Yes in the drop-down list.
Accreditation Category Selection block - Select Yes next to either Category I animals or Category II animals. Select No for the other category.
Accreditation Renewal block - For each training course, enter the appropriate Course Title and Date Completed information.
To switch from Category I animals to Category II animals, you must enter one or more courses that total 6 units.
To switch from Category II animals to Category I animals, you must enter one or more courses that total 3 units.
Verify that you have filled out every required field on the form.
Click the Save New 1-36A Form button.
This message appears: The veterinarian record was saved.
Click the Return to Find Vet Results button. In the results list, this veterinarian now shows this change in the Category column.
Enter a veterinarian's accreditation renewal data - Fill out only these fields:
Accreditation Renewal - Select Yes in the drop-down list.
A pop-up message asks if you want to proceed. Click OK.
Accreditation Renewal block - For each course, enter the appropriate Course Title and Date Completed information.
To renew a Category I animals accreditation, enter one or more courses that total 3 units.
To renew a Category II animals accreditation, enter one or more courses that total 6 units.
Authorization Date - Make sure this field is blank.
Renewal Date - Is auto-filled by VSPS with a date three years from the date that all required fields on the VS 1-36A Form were completed.
Verify that you have filled out every required field on the form.
Click the Save New 1-36A Form button.
This message appears: The veterinarian record was saved.
Enter a veterinarian's post-revocation re-accreditation data - Fill out only these fields:
Post-Revocation Re-accreditation Renewal - Select Yes in the drop-down list.
Verify the veterinarian's data that appears in the Personal Information, Accreditation Category Selection, and Contact Information blocks.
Verify the veterinarian's data in the Species Category *, Species Codes *, Primary Medical Discipline, and Employment Type * fields.
Authorization Date - Make sure this field is blank.
Renewal Date - Is auto-filled by VSPS with a date three years from the date that all required fields on the VS 1-36A Form were completed.
Verify that you have filled out every required field on the form.
Click the Save New 1-36A Form button.
This message appears: The veterinarian record was saved.
Update a veterinarian's contact information - Fill out only these fields:
Contact Information Change - Select Yes in the drop-down list.
Personal Information block (edit, as needed).
Check if your name has changed - Select the appropriate value in the drop-down list.
Name of Veterinarian (Last, First, M, Suffix) * - Name of the veterinarian.
Six Digit National Accreditation Number - Unique, VSPS-assigned number. This number is assigned after a veterinarian has submitted his Elect to Participate and Initial Accreditation data.
Other Names Used (e.g. Maiden Name) - Alternate, maiden, birth, or nicknames for this veterinarian. Enter an alternate last name in the first blank field and/or an alternate first name in the second blank field.
Date of Birth * - Date on which the veterinarian was born.
School of Veterinary Medicine * - School from which the veterinarian graduated. (Note: If the school name does not appear in the drop-down list, click
to open a search box.)
Year Graduated * - Year in which the veterinarian graduated from veterinary school.
State where Orientation Completed * - State in which the veterinarian completed his orientation training.
Date Orientation Completed * - Date on which the veterinarian completed his orientation training.
Are you interested in participating in State or Federal agriculture emergency response efforts? - Select the appropriate value in the drop-down list.
Contact Information block / Home Address section (edit, as needed).
Address * - Physical address information for the veterinarian's residence.
County * - County in which the veterinarian's residence is located. May be auto-filled if you first enter the State and Postal Code values, and VSPS can validate them. (Note: If the county name does not appear in the drop-down list, click
to open a search box.)
Country * - Country in which the veterinarian's residence is located.
Home Phone * - Primary telephone number for the veterinarian's residence.
Email Address * - Email address for the veterinarian's residence.
City * - City in which the veterinarian's residence is located.
State * - State which the veterinarian's residence is located.
Postal Code * - Zip code for the veterinarian's residence. (Note: You can enter this value first, then press TAB. The City, State, and County fields are then auto-filled by VSPS.)
If your contact information is the same as your business information, may it be released to the public by the USDA? - Click Yes if the veterinarian wants his home/business contact information to be searchable and viewed on the VSPS Web site. Click No if the veterinarian does not want this information made available.
Contact Information block / Primary Business Address section (edit, as needed). (Note: Only one primary business address is needed, no matter how many States a veterinarian is authorized to practice in.)
Is Primary Business Address SAME as Home Address? - If you click Yes, VSPS automatically copies the appropriate data from the Home Address fields into this section's fields.
Business Name - Name under which the veterinarian's business operates.
Address * - Physical address information for the veterinarian's business.
County * - County in which the veterinarian's business is located. May be auto-filled if you first enter the State and Postal Code values, and VSPS can validate them. (Note: If the county name does not appear in the drop-down list, click
to open a search box.)
Country * - Country in which the veterinarian's business located.
Business Main Phone * - Primary telephone number for the veterinarian's business.
Business Fax - Fax machine number at the veterinarian's business.
Business Cell Telephone - Cell number for the veterinarian's business.
City * - City in which the veterinarian's business is located.
State * - State which the veterinarian's business is located.
Postal Code * - Zip code for the veterinarian's business. (Note: You can enter this value first, then press TAB. The City, State, and County fields are then auto-filled by VSPS.)
May your business contact information be released to the public by the USDA? - Click Yes if the veterinarian wants his business contact information to be searchable and viewed on the VSPS Web site. Click No if the veterinarian does not want this information made available.
Authorization Date - Make sure this field is blank.
Verify that you have filled out every required field on the form.
Click the Save New 1-36A Form button.
This message appears: The veterinarian record was saved.
Click the Return to Vet Record button.
In the veterinarian record, verify that your changes appear on the Main tab.
