List of Archived Forms
This page lets you view a list of VS 1-36A Forms that have been submitted and attached to a veterinarian record.
If your VSPS role is for a veterinarian, you can view the list of VS 1-36A Forms:
On the VSPS Home page, log into VSPS.
From the left menu, select Vet Accreditation.
In the Vet Accreditation Module, click My Veterinarian Record.
The My Veterinarian Record page appears and displays your veterinarian record.
In the Veterinarian Summary block, click [View Form 1-36A PDF History].
The List of Archived Forms page appears. It contains a summary list of all VS 1-36A Forms that have been attached to your record.
To see a specific VS 1-36A Form file, click [View] in the Action column next to that form's name. Each VS 1-36A Form file may have one or more items in it, such as a printable letter or certificate, a mailing label, and a printable 1-36A form.
A pop-up window displays the form file.
To close this window, click the X in the upper-right corner.
If your VSPS role is not for a veterinarian, you can view the list of VS 1-36A Forms:
On the VSPS Home page, log into VSPS.
From the left menu, select Vet Accreditation.
In the Vet Accreditation Module, click Find Vet.
Use the Find Vet page to enter search criteria; then click the Search button to execute a search for a veterinarian.
In the search results list appears, click [Edit] in the Action column next to the veterinarian whose record you want to view.
The Edit Veterinarian Record page appears and displays the selected veterinarian record.
In the Veterinarian Summary block, click [View Form 1-36A PDF History].
The List of Archived Forms page appears. It contains a summary list of all VS 1-36A Forms that have been attached to a veterinarian record.
To see a specific VS 1-36A Form file, click [View] in the Action column next to that form's name. Each VS 1-36A Form file may have one or more items in it, such as a printable letter or certificate, a mailing label, and a printable 1-36A form.

To close this window, click the X in the upper-right corner.
