VS 1-27 Data Entry
To create a Permit for Movement of Restricted Animals Form (VS 1-27), you need to first create an Importation Request, VS 17-29 Form, and VS 17-30 Form. VSPS then gives you the ability to create and attach a VS 1-27 Form to the VS 17-30 Form.
A VS 1-27 Form reports data on a single species. If your importation is for multiple species, you need to create a separate pair of VS 17-30 and VS 1-27 Forms for each species.
NOTES:
Fields marked with an asterisk (*) are required.
Some drop-down lists allow you to select multiple items by pressing and holding the CTRL key while highlighting each item.
Dates can usually be entered in two ways: using the Calendar
, or entering them manually in the mm/dd/yyyy format.
Phone numbers: VSPS defaults to the U.S. country code of (1). If you are entering:
a U.S. number, follow the country code of (1) with ten digits.
a non-U.S. number, be sure to include the correct country code.

How to create/edit an Importation Request and VS 17-29 Form:
On the VSPS Home page, log into VSPS.
From the left menu, select Animal Import. The Animal Import Module displays.
Select Routine Import. The Routine Import Module displays.
Select one of the tasks below:
To start a new importation request, click Create New Importation. Complete the Create Importation Request page, then click the Create button.
To find and open an existing importation record, click Find Existing Importation. Enter your search values, then click the Search button. When the results list appears, click [Edit] next to the record you want.
The Importation Request Summary page displays, with three sections in it: Importation Request Summary, Animal Groups, and Form Management.
Add one or more animal groups to this importation by clicking the Create Animals button in the Animal Groups section. The Define Animals page displays in which you enter your enter your animal data.
Use the Create Group, IDs, Genders, or Upload command to enter more data for each animal group you are adding to the Importation Request.
Return to the Importation Request Summary page.
How to create/edit a VS 17-30 Form:
On the Importation Request Summary page, in the Form Management section, click [New] or [Edit] next to Importation Release (17-30). The 17-30 Data Entry page displays.
Complete the 17-30 Data Entry and Released Animals sections, then click the Create or Save button.
Click the Return button to go back to the Importation Request Summary page.
How to create/edit a VS 1-27 Form:
On the Importation Request Summary page, in the Form Management section, click [New] or [Edit] next to Movement of Restricted Animals (1-27). The Form 1-27 Data Entry page displays.
The Consignor, Consignee, and Owner sections may be auto-filled with certain data from the Importation Request and VS 17-30 Form. Change these values, as needed. Complete any blank fields in these sections, as appropriate.
Enter the following:
Moved from (name) * - Name and location of premises that the animals originated from (if different than the location displayed in the Consignor section).
Moved from (location) * - Name and location of the destination premises that the animals are being shipped to (if different than the location displayed in the Consignee section).
State Where Issued - State in which the Port of Arrival is located.
Movement To Be * - Type of movement for this animal shipment.
Interstate - Movement from one State to another State within the U.S.
Intrastate - Movement within the boundaries of a single State within the U.S.
Movement For * - Purpose for moving this animal shipment.
Disease - Disease associated with this animal shipment.
Status of Animals:
No. Reactor - Number of animals in the shipment that are classified as reactor animals for a specific disease.
No. Exposed - Number of animals in the shipment that are classified as exposed animals for a specific disease.
No. Other (specify) - Number of animals in the shipment that are classified as diseased, dead, etc. for a specific disease.
Other status - Number of animals in the shipment that have other types of restricted status.
Status of Herd of Origin - Status of the Herd of Origin to which this animal shipment belonged.
Status of Area of Origin - Disease program status of the Area in which the Herd of Origin was located.
Vehicle Identifiers - Identification of the vehicle(s) used to transport the animal shipment.
Seals * - Seal numbers of the containers used to transport items/animals in this importation. Enter one seal number per line.
Disinfection Required * - Check this box if the vehicle transporting the animal shipment requires disinfection at the destination.
In the Signatures section, complete only the fields listed below:
Inspector Signature:
Name - Name of the inspector who examined the animal shipment.
Date - Date on which the inspector examined the animal shipment.
Time - Time when the inspector released the animal shipment. Use this format - hh:mm. Example: 08:45.
Void After:
Date * - Date when the animal shipment should arrive. (Note: In the VS Form 1-27 that documents the first leg of a multi-leg animal shipment, this value should be the estimated transit time for the entire trip. In additional VS Form 1-27s that document later legs in the trip, this value should be the transit time for just that leg, not for the entire trip.)
Time * - Time when the animal shipment should arrive. Use this format - hh:mm. Example: 08:45. (Note: See the Note for the Date field directly above.)
Shipper/Owner Signature:
Name - Name of the shipper/owner of the animals.
Title - Title used by the shipper/owner of the animals.
Date - Date on which the shipper/owner of the animals signs the VS 1-27 Form.
Disinfection Inspector Signature:
Name - Name of the disinfection inspector.
Title - Title used by the disinfection inspector.
Date - Date on which the disinfection inspector signed the VS 1-27 Form.
Click the Create button.
These messages appear: Importation Request has been updated. VS1-27 Form has been created.
As needed, enter additional data or modify existing data on this form. Be sure to click the Save button to save your recent changes.
When ready to make this form available to others to enter their data, click the Submit button.
This message appears: VS1-27 Form has been updated.
Click the Return button to go back to the Importation Request Summary page.
In the Form Management Section, you may be able to apply any of these commands to the VS 1-27 Form:
[Add Leg] - Click to display a Form 1-27 Data Entry page, on which you can enter data for the next segment of the animal shipment's journey. Example: Your animal shipment must make one additional stop between the port of arrival and the final destination. In Steps 2-3 above, you would have entered data about the first leg, which is the animal shipment arriving at the port. Clicking on [Add Leg] lets you now enter data for the next trip segment from the port to a middle location. You would then click [Add Leg] one more time to enter the data from the middle location to the final destination.
[Edit] - Click to edit any previously-entered data on this form.
[Preview] - Click to view and/or print the submitted .pdf form content.
[Void] - Click to void/cancel the form.
How to reconcile a VS 1-27 Form:
On the VSPS Home page, log into VSPS.
From the left menu, select Animal Import. The Animal Import Module displays.
Select Routine Import. The Routine Import Module page displays.
On this page is an Importation Requests with expired VS 1-27's table.
The forms shown in red font are VS 1-27 Forms for animal shipments that did not arrive at a port within the designated transit time period.
The forms shown in blue font are 1-27 Forms for pending animal shipments that will be arriving at a port.
Click [Edit] under the Action column next to the record you want to reconcile. The Importation Request Summary page that this VS 1-27 is associated with will display.
Under the Form Management section, click [Edit] next to Movement of Restricted Animals (1-27). Note: If an animal shipment made multiple stops after arriving in the U.S., you will need to reconcile and close the VS 1-27 for each leg of the shipment's journey.
On the Form VS 1-27 Data-Entry page, complete the following fields only in the Signatures section:
Place Animals Received - Name of the destination facility receiving the animals.
Date Animals Arrived * - Date on which the animal shipment arrived at the destination facility.
No. Animals Received - Total number of animals in the shipment that was received.
Date Slaughtered/Quarantined - Date on which the animals in the shipment were either slaughtered or quarantined.
Seals Broken:
Date * - Date on which the seals of the containers used to transport the animals was broken.
Time * - Time when the seals on the containers were broken. Use this format - hh:mm. Example: 08:45.
Click the Save button to save your changes.
This message appears: VS1-27 Form has been updated.
When ready to reconcile this form, click the Close button.
This message appears: VS1-27 Form has been updated.
Click the Return button to go back to the Importation Request Summary page.
In the Form Management Section, under the Movement of Restricted Animals (1-27) document, you will see the VS 1-27 Form with a Closed status. You may still be able to apply any of these commands to this closed VS 1-27 Form or to any other VS 1-27 Form shown:
[Add Leg] - Click to display a Form 1-27 Data Entry page, on which you can enter data for the next segment of the animal shipment's journey.
[Edit] - Click to edit any previously-entered data on this form.
[Preview] - Click to view and/or print the original .pdf form content.
[Void] - Click to void/cancel the form.
