VS 17-8 Data Entry

The VS 17-8 (Agreement of Pet Bird Owner) requires specific information and is completed at the time of entry by the Port VMO.  A VS 17-30 (Report of Animals, Poultry, or Eggs Offered for Importation) or VS 17-30R (Importation Refusal) must be created before the VS 17-8.

To access a VS 17-8:

  1. On the VSPS Home page, log into VSPS.

  2. From the left menu, select Animal Import.

  3. Select Routine Import.

  4. Select Find Existing Importation > enter criteria > Search > [Edit]  or

Select Create New Importation > enter data > Create.  

The Importation Request Summary displays.

  1. Under the Form Management section all of the available forms for this importation are displayed.  

  2. Click [New] beside the Pet Bird Owner Agreement (17-8) title.  The 17-8 Data Entry page displays.

To complete a VS 17-8:

  1. Enter the following Form 17-8 Header information:

  1. Enter the following Owner contact information for the bird(s) being imported:

NOTE: Either Business Name or First and Last Name are required.

   NOTE:  For foreign states, select the Country first.

NOTE:  VSPS defaults to the U.S. country code of (1) that must be followed by a number with 10 digits; if you are entering a non-U.S. number you must enter the corresponding country code.

  1. Enter Form 17-8 Disposition information - The balance of this form has dynamic fields that display only the fields required depending on the disposition or section that is selected.  Click one of the four sections (A, B, C, or D) that describes the disposition of these birds:

[A] Bird(s) to remain in owner's possession and separate from all other birds.

This section is used for US origin pet/performing birds or exhibition poultry returning to the US from anywhere other than Canada (i.e. those birds requiring home quarantine).

[B] Bird(s) are from Canada or have been outside the U.S. less than 60 days.

This section is used only for pet/performing birds or exhibition poultry from Canada, or US origin birds returning from Canada (i.e. birds not requiring home quarantine).

[C] Bird(s) to be exported to another country.

This section is used to re-export any birds showing clinical signs of disease at a port of entry or to document the agreement of the importer to re-export birds otherwise found not eligible for entry due to smuggling or action taken by CBP preventing the owner from continuing into the US.  Unless seized by CBP, pet/performing birds or exhibition poultry not eligible for entry should be permitted to be re-exported whenever possible.

[D] Bird(s) to be abandoned.

This section is used when birds are abandoned to either CBP or VS at the port of entry.

Enter the following contact information for the Location Birds Held (can be same as Owner), and for the Federal Official to Contact (usually the AVIC in the State where home quarantine will take place; can use Address Book to populate fields if data has been entered into it):
