VS 17-30 Data Entry, Release Information and Access to VS 17-33, VS 17-130
The Report of Animals, Poultry or Eggs Offered for Importation Form (VS 17-30) requires specific and final importation information to be completed at the time of entry by the Port VMO. When an importation is released, it is either released to continue to its destination or released to one of the following forms:
Animals Imported for Immediate Slaughter Form (VS 17-33) - This form accompanies the animals to the slaughter facility. At the facility, the Federal or State Inspector enters data on this form, then returns the form to the Port.
Ruminants Imported to Designated/Approved Feedlots Form (VS 17-130) - This form accompanies the animals to the designated or approved feedlot. At the feedlot, the VMO enters data on this form, then returns the form to the Port.

NOTES:
Fields marked with an asterisk (*) are required.Some drop-down lists allow you to select multiple items by pressing and holding the CTRL key while highlighting each item.
Dates can usually be entered in two ways: using the Calendar
, or entering them manually in the mm/dd/yyyy format.
Phone numbers: VSPS defaults to the U.S. country code of (1). If you are entering:
a U.S. number, follow the country code of (1) with ten digits.
a non-U.S. number, be sure to include the correct country code.
How to create a VS 17-30 Form:
On the VSPS Home page, log into VSPS.
From the left menu, select Animal Import. The Animal Import Module displays.
Select Routine Import. The Routine Import Module page displays.
Select one of the tasks below:
To start a new importation request, click Create New Importation. Complete the Create Importation Request page, then click the Create button.
To find and open an existing importation record, click Find Existing Importation. Enter your search values, then click the Search button. When the results list appears, click [Edit] under the Action column next to the record you want.
The Importation Request Summary page displays, with three sections in it: Importation Request Summary, Animal Groups, and Form Management.
If needed, add one or more animal groups to this importation by clicking the Create Animals button in the Animal Groups section. The Define Animals page displays in which you will enter your animal data.
Return to the Importation Request Summary page.
Under the Form Management section, click [New] next to Importation Release (17-30). The 17-30 Data Entry page displays.
Complete the 17-30 Data Entry section:
Date of Issuance * - Date on which the animals are released. Defaults to the current date. You can change this value.
Save Time of Issuance - Defaults to the time that the VS 17-30 is saved. Check this box to have this information recorded in the importation.
CBP Code - Customs and Border Protection code for this importation.
Alt Number - An alternative Customs and Border Protection code for this importation.
Seals * - Seal numbers of the containers used to transport items/animals in this importation. Enter one seal number per line.
Remarks - Supplemental information that is pertinent to this importation.
Complete the Released Animals section, using either method below:
Click the Release All button to release all animals in all the groups shown. The total numbers appear in the Number to Release fields.
Manually complete the Number to Release field next to each animal group.
Click the Create button.
This message appears: VS 17-30 Form has been created.
You can now choose to attach a VS 17-33 or VS 17-130 Form to the just-created VS 17-30 Form. (Note: Keep in mind that once a VS 17-33 or VS 17-130 Form has been created and attached, you can delete it only by voiding the VS 17-30 that it was created with.)
In the Attached Forms section, check the radio button next to an option:
None - Your VS 17-30 does not require a VS 17-33 or VS 17-130 Form to be attached to it.
Form 17-33 - Displays the data-entry fields that are specific to a VS 17-33 Form.
Form 17-130 - Displays the data-entry fields that are specific to a VS 17-130 Form.
How to create/attach/reconcile a VS 17-33 Form:
(Note: The first time you open a VS 17-33 Form, you see that many of the fields are auto-filled with data from both the Importation Request page and the VS 17-29 Form. These auto-filled fields may or may not be editable.)
If necessary, reopen the VS 17-30 Form that you want to attach a VS 17-33 Form to.
In the Attached Forms section, check the radio button next to Form 17-33. A blank Form 17-33 appears.
On the Form 17-33, complete the following sections only:
Consignor section - Contact information for the seller. You can change these values using Method A or Method B below:
Method A...Click the
icon to search for a contact record in your Address Book. You then copy the record into this form.
Method B...Manually enter any changes into the following fields only (if they appear in this section).
Business Name - Formal name of this contact's business. (Note: Enter this value OR a First Name and Last Name.)
First Name - First name of this contact.
Last Name - Last name of this contact.
Professional Title - Professional or career-related title for this contact.
Email Address - Email address for this contact.
Address 1 * - Physical address location where this contact works or lives.
Address 2 - Additional address information.
City * - City where this contact works or lives.
State * - State where this contact works or lives. (Note: For foreign states, select the Country first.)
Postal Code * - Postal code for this contact's address.
Country * - Country where this contact works or lives.
Phone * - Phone number where this contact works or lives.
Fax - Facsimile machine contact number where this contact works or lives.
To save this revised contact record to your Address Book, click on
.
Consignee section - Contact information for the animals' new owner.
(Note: For help on the fields in this section, see the Consignor section shown above.)
Port Veterinarian section - Contact information for the Veterinary Medical Officer (VMO) at the Port of Arrival who is releasing the shipment.
(Note: For help on the fields in this section, see the Consignor section shown above.)
Establishment section - Contact information for the facility that is receiving the shipment.
(Note: For help on the fields in this section, see the Consignor section shown above.)
Veterinarian at Destination section - Contact information for the Veterinarian at the destination facility.
(Note: For help on the fields in this section, see the Consignor section shown above.)
Click the Save button to attach this form to the VS 17-30 Form. This message appears: VS17-33 Form has been created.
When ready to make this form available to others to enter their data into it, click the Submit 17-33 button.
This message appears: VS17-33 Form has been updated.
To reconcile this form, complete the following fields and tasks only:
GPS Coordinates - Geographic coordinates for the slaughter facility's physical location.
Date of Slaughter * - Date on which the animals in the shipment are slaughtered at the facility.
Number of Animals Received * - Total count of the animals in the shipment that is delivered to the facility.
Tag Number - Tag number for the animals in the shipment.
Description of Animals - General comments about the animals' conditions.
TB Lesions - Observations of any tuberculosis lesions on the animals.
Date of Signature * - Date on which the VS 17-29 is signed.
Vehicle Identifiers - Identification of the vehicle(s) used to transport the animal shipment.
Remarks - Supplemental information that is pertinent to this animal shipment.
Make sure the Establishment section is filled out.
Click the Save button. This message appears: VS17-33 Form has been updated.
Click the Close 17-33 button. This message appears: VS17-33 Form has been updated.
If the closed VS 17-33 Form is the only record associated with the VS 17-30 Form, you can now close the VS 17-30 Form. Click the Close 17-30 button. This message appears: VS17-30 Form has been updated.
Click the Return button to go back to the Importation Request Summary page. In the Form Management section, under the Importation Release (17-30) document, you will see the 17-33 document with a Closed status.

Click the Save button to attach this form to the VS 17-30 Form. This message appears: VS17-130 Form has been created.
When ready to make this form available for others to enter their data into it, click the Submit 17-130 button.
This message appears: VS17-130 Form has been updated.
To reconcile this form, complete the following fields and tasks only:
Date Received * - Date on which the VS 17-29 is signed.
Are Seals Intact - Check this box if the answer is "Yes".
Was Vet Contacted - Check this box if the answer is "Yes".
Vehicle Identifiers - Identification of the vehicle(s) used to transport the animal shipment.
Remarks - Supplemental information that is pertinent to this animal shipment.
Responsible Individual Signature:
Name - Name of the individual signing this form.
Title - Title of this individual.
Date - Date on which the VS 17-29 is signed.
Make sure the Establishment section is filled out.
Click the Save button.
Click the Close 17-130 button. This message appears: VS17-130 Form has been updated.
If the closed VS 17-130 Form is the only record associated with the VS 17-30 Form, you can now close the VS 17-30 Form. Click the Close 17-30 button. This message appears: VS17-30 Form has been updated.
Click the Return button to go back to the Importation Request Summary page. In the Form Management section, under the Importation Release (17-30) document, you will see the 17-130 document with a Closed status.
