VS 17-29 Data Entry

The Broker or Port VMO and staff can create the VS FORM 17-29; Declaration of Importation (for Animals, Animal Semen, Animal Embryos, Birds, Poultry, or Hatching Eggs). Most fields are auto-filled by VSPS, from the information that was entered into the Importation Request.

Dates can usually be entered in two ways: using the calendar , or typing them manually using mm/dd/yyy format.
Fields marked with (*) are required.

To access the VS 17-29:

On the VSPS Home page, log into VSPS.
From the left menu, select Animal Import.
Select Routine Import.
Select Find Existing Importation > enter criteria > Search > [Edit] or

Create New Importation > enter data > Create or

The Importation Request Summary screen displays.

NOTE: A Port VMO/Support Staff can also find a 17-29 using View My Importations > Available Quick Search: Submitted 17-29 for my port as the port of entry.

Under the Form Management section, all of the available forms for this importation are displayed.
Click [New] or [Edit] next to the Importation Declaration (17-29) title. The 17-29 Data Entry page displays.

NOTE: Click [Preview] to see the pdf file of the form with content, or click [Void] to cancel the form.

To enter information regarding the person who declares that the content of the VS 17-29 is true and correct:

Enter the following:

Name * } Name of the Importer or Broker that is going to sign this form.
Title * } Title of the person signing this form.
Date * } Date the VS 17-29 was signed.
Remarks } Comments that are relevant to this importation. These comments will be placed on the VS 17-29 in the remarks field. Data you may want displayed are CBP or HTC codes.

Click the Create and/or Save button.
Click the Return button to go back to the previous screen.
