VS 17-129 Data Entry
Before certain animals (equine from a CEM country...) are imported into the United States, the Broker must fill out and make available a VS FORM 17-129: Application for Importation or In Transit Permit. VSPS stores the data as it is collected, thereby eliminating the need to re-enter information throughout the animal importation process.

NOTES:
Dates can usually be entered in two ways: using the calendar
, or typing them manually using mm/dd/yyy format.
Fields marked with an asterisk (*) are required.

To access the 17-129:
On the VSPS Home page, log into VSPS.
From the left menu, select Animal Import.
Select Routine Import.
Select Find Existing Importation > enter criteria > Search > [Edit] or
Select Create New Importation > enter data > Create.
The Importation Request Summary displays.
Note: View My Importations is also be available for certain roles. If you click on it, Available Quick Searches display. You can click on the link: Open Recent Importation Requests with 17-129 Pending State Permit and Destination in my Area. Click [View] next to the appropriate Importation Request, and [Edit] under Application for Permit (17-129) to access the form's data entry screen. Click [Preview] to view the form.
Under the Form Management section all of the available forms for this importation are displayed.
Click [New] beside the Application for Permit (17-129) title. The 17-129 Data Entry page displays.
To create or edit a 17-129:
Enter information into the following fields.
Request Status - Status of the Importation Request (IR); defaults to Initial. Change the status as appropriate (Pending State Permit, Approved State Permit, Pending Federal Permit, Approved Federal Permit).
Route of Travel - List all ports that are involved in the transport of the animals, semen, embryo, or hatching eggs. This information will populate to section 7 of the Application For Import or In Transit Permit form.
Waiver Type - From the drop-down menu, select a waiver type if appropriate (60 Day Re-entry, Gelding, Racing Thoroughbred, Spanish Pure Breed, Wild or Zoo Animal).
Transportation Mode - From the drop-down menu, select the type of transportation that will be used: Land, Air, Sea or Rail.
Shipping Date - Date animals were shipped.
Remarks - Enter remarks pertinent to this importation. These remarks will populate to section 13 on the Application For Import or In Transit Permit form.
Signature of Importer:
Name - Name of the Importer or Broker that is going to sign this form.
Title - Title of the individual signing this form.
Date - Date the VS 17-129 was signed.
Animals Needing Permit:
Click [+] to show individual animals.
Select All - Check the box if you want to select all animals in the list.
Select individual animals - Check the box next to an individual animal.
Note: If check marks already appear in some boxes, it indicates that the animal in that row needs a permit; enter a permit number into the permit # field.
Click Create or Save.
