VS 10-4 Data Entry
The Specimen Submission Form (VS 10-4) requires specific information and is completed at the time of entry by the Port VMO.
NOTES:
Fields marked with an asterisk (*) are required.
Some drop-down lists allow you to select multiple items by pressing and holding the CTRL key while highlighting each item.
Dates can usually be entered in two ways: using the Calendar
, or entering them manually in the mm/dd/yyyy format.
Phone numbers: VSPS defaults to the U.S. country code of (1). If you are entering:
a U.S. number, follow the country code of (1) with ten digits.
a non-U.S. number, be sure to include the correct country code.

How to create a VS 10-4 Form:
On the VSPS Home page, log into VSPS.
From the left menu, select Animal Import. The Animal Import Module displays.
Select Routine Import. The Routine Import Module page displays.
Select one of the tasks below:
To start a new importation request, click Create New Importation. Complete the Create Importation Request page, then click the Create button.
To find and open an existing importation record, click Find Existing Importation. Enter your search values, then click the Search button. When the results list appears, click [Edit] under the Action column next to the record you want.
The Importation Request Summary page displays, with three sections in it: Importation Request Summary, Animal Groups, and Form Management.
If needed, add one or more animal groups to this importation by clicking the Create Animals button in the Animal Groups section. The Define Animals page displays in which you will enter your animal data.
Return to the Importation Request Summary page.
In the Form Management Section, click [New] next to Specimen Submission (10-4).
On the Form 10-4 Data-Entry page, enter the following:
Number in Herd * - Number of animals in the herd that specimens were collected from. Is auto-filled by VSPS with data from the Importation Request.
Number Affected * - Number of animals in this herd that showed symptoms.
Number Dead * - Number of dead animals in this herd.
Examinations Requested - Names of the examinations to be performed on the specimens.
Collected by Name - Name of the person who collected the specimens. Is auto-filled by VSPS with the name of the port's Veterinary Medical Officer (VMO). You can change this value.
Date Collected - Date on which the specimen was collected. Is auto-filled by VSPS with today's date. You can change this value.
Authorized By Name - Name of the person authorizing the specimen collection. Is auto-filled by VSPS with the name of the port's Veterinary Medical Officer (VMO). You can change this value.
Submission Purpose - Reason why the specimens are being tested.
Referral Number - Document Number. Is auto-filled by VSPS.
Preservation Method(s) - Method used for preserving the specimens during shipment to the testing laboratory. You can select multiple methods (see the NOTES at the beginning of this help topic).
If other, specify - Any preservation method not displayed in the drop-down list. Type the method in this field.
Specimen Type(s) - Type of specimens you are sending in for testing. You can select multiple types (see the NOTES at the beginning of this help topic).
If other, specify - Any specimen type not displayed in the drop-down list. Type this specimen type in this field.
Number Of Specimens * - Number of specimens being sent for testing.
Specimen Source - Source from which the specimens were obtained.
If other, specify - Any animal source not displayed in the drop-down list. Type this animal source in this field.
Number Of Animals Sampled * - Number of animals that specimens were collected from.
Animal Location State - State in which the animals were located when they were sampled.
Animal Location County - County in which the animals were located when they were sampled. (Note: Fill out the Animal Location State field first in order to customize the drop-down list for this Animal Location County field.)
Remarks - Additional information that is pertinent to this specimen submission (such as history, clinical signs, postmortem findings, and tentative diagnosis).
Submitter Signature - Enter the following:
Name - Name of the person submitting this form. Is auto-filled by VSPS with the name of the port's Veterinary Medical Officer (VMO). You can change this value.
Date - Date on which this form is created. Is auto-filled by VSPS with today's date. You can change this value.
Submitter Section - Information about the person submitting this form. May be auto-filled by VSPS with data from the Importation Request. You can change these values using Method A or Method B below:
Method A...Click the
icon to search for a contact record in your Address Book. You then copy the record into this form.
Method B...Manually enter any changes.
Business Name - Name of the submitter's business.
First Name - First name of this contact.
Last Name - Last name of this contact.
Address 1 * - Address for this contact.
Address 2 - Additional address information for this contact.
City * - City in which this contact works/lives.
State * - State in which this contact works/lives. (Note: For foreign states, select the Country value first.)
Postal Code * - Postal code for this contact.
Country * - Country in which this contact works/lives.
Phone - Phone number for this contact.
Fax - Fax number for this contact.
To save this revised contact record to your Address Book, click on
.
Owner Section - Information about the animal owner. May be auto-filled by VSPS with data from the Importation Request.
(Note: For help on the fields in this section, see the Submitter section shown above.)
Animals to Test Section: Animals whose specimens are to be tested. Is auto-filled by VSPS with data from the Importation Request.
Click on the + to expand an animal group record and see all the animals in it.
In the Test column, check the box next to each animal whose specimen should be tested.
In the Sample Number column, enter the Sample Number that should be assigned to a animal's sample.
NVSL Information Section: Do not fill out this section.
Click the Create button.
This message appears: VS10-4 Form has been created.
As needed, enter additional data or modify existing data in this form. Be sure to click the Save button to save your recent changes.
When ready to make this form available to others to enter their data, click the Submit button.
This message appears: VS10-4 Form has been updated.
Click the Return button to return to the Importation Request Summary page.
In the Form Management Section, you may be able to apply any of these commands to the VS 10-4:
[Edit] - Click to edit any previously-entered data on this form.
[Preview] - Click to view and/or print the submitted .pdf form content.
[Void] - Click to void/cancel the form.
How to reconcile a VS 10-4 Form:
On the VSPS Home page, log into VSPS.
From the left menu, select Animal Import. The Animal Import Module displays.
Select Routine Import. The Routine Import Module page displays.
To open an existing importation record, click Find Existing Importation. Enter your search values, then click the Search button. When the results list appears, click [Edit] under the Action column next to the record you want.
In the Form Management Section, click [Edit] next to Specimen Submission (10-4).
On the VS 10-4 Data-Entry page, complete the NVSL Information section only:
Condition - Condition of the specimen.
Priority - Testing priority of the specimen.
Distribution - Distribution method for communicating the test results.
Received By Name - Name of the person receiving the specimens.
NVSL Accession Number - NVSL accession number.
Click the Save button to save these changes.
This message appears: VS10-4 Form has been updated.
When ready to reconcile this form, click the Close button.
This message appears: VS10-4 Form has been updated.
Click the Return button to go back to the Importation Request Summary page.
In the Form Management Section, under the Specimen Submission (10-4) document, you will see the VS 10-4 Form with a Closed status. You may be able to apply any of these commands to this closed VS 10-4 Form:
[Edit] - Click to edit any previously-entered data on this form.
[Preview Original] - Click to view and/or print the original .pdf form content.
[Preview Amended] - Click to view and/or print the current .pdf form content.
[Void] - Click to void/cancel the form.
[Audit] - Click to view the change history of the form.
